Double Flush mid-fuse Rivets

Trying to figure this out. Like many have said, the fuse is exponentially more challenging than the wings or tail.

There appears to be five double flush rivets on each side of the aft fuselage. I get the reason why, but not sure how to make it happen. The instruction say the shop head goes on the inside. The carry through spar is too thick to dimple. So do I machine countersink the top side of the spar flange and the bottom skin? Not sure how else to do that.

For what it's worth, I didn't end up making these double flush. I just dimpled them like all of the other flange holes. I know you're supposed to make them double flush to allow them to fit under the gear tower, but I put the towers in place and measured. My towers were really close, but I had enough room without making double flush rivets. You don't want to just blindly do it without verifying with your own parts though.
