Seat belt attach point fifth belt

I lost the instructions for installing the five point seat belts provided from vans.
I already had installed the brackets and stuff under the seats but now I want to install the belts and I can not figure how the belt is to fastened or secured.
Can someone post a link to a photo or explain how to do this?

Any chance we could get a measurement for the spread while using the hooker harness?

5/16" spread for the Hookers and a 1/4" bolt!

Van has a 1/8" spread and an AN5 bolt!

P.S. for the drilling and fit up I clamped spacers in between the attach brackets and wiggled them around until perfect. Give that a try.
