Program Letter

Could someone tell me what changes on the RV12 Program Letter from the standard homebuilt letter. San Antonio MIDO is cooperative but unsure on this and we all know how effective a document already accepted somewhere else can be. Also, Vans is listed on the data plate as manufacturer, right?
