Aircraft airframe log verbage

I am trying to get ready for the DAR. Can anyone give me the verbage that they used for their airframe log so I can get an idea how to word mine. We have no experimentals in the area & am in the dark as to the proper wording.

"I certify that I built this aircraft for my own education and recreation. I have inspected it and consider it eligible for an experimental airworthiness certificate for the purpose of operating amateur-built aircraft under the provisions of FAR 21.191g." Signed and dated.
I provide a pre-typed statement on a label for my applicants. Your DAR may do the same.
 
My DAR furnished all the information needed
for the log books. This included the entry
I had to make at the end of phase-1.
Ask your DAR if he furnishes the info.

What to write?

What would be the verbage for an oil change or some other maintenance we perform on our aircraft as the repairmen? I just received my repairman?s certificate for my RV4 and I am not really sure what to document in my airframe logbook or how to say it.
