Condition Inspection Checklist in Word or Excel format & Logbook wording

Does anyone have the Condition Inspection Checklist below in Word or Excel format? I like the checklist but there's a few things I would like to change to apply to my airplane without having to retype the whole thing.

I also want to make sure that the checklist hits on all the points outlined in FAR 43 Appendix D because I intend to use the phrase quoted in post #2 on this EAA forum.

Mike covered the check list part... so a comment on the log book part.

Your plan will work, but the best plan would be to open the operating limitations for your airplane and see what they require for the log book entry. If nothing else, to get familiar with what is in there. They are the rule book governing specific operating requirements of you airplane (and they are not necessarily the same for everyones).
 
Log book

I can't disagree with checking you Op Lim's but this is the wording I use in my log book:

"I certify that this aircraft has been inspected In Accordance With the Scope and Detail of FAR Part 43, App. D and found to be in condition for safe operation."

The entry need to be signed, dated. Also print your name and "authorization" including certificate number. Your authorization is probably "Repairman" (if you were an A&P or IA, you wouldn't be asking this question)

The entry should also include Total Time in Service.
