The difference is:

A "TSO" Sticker and an FAA 8130....and a couple hundred bucks!

My A200 (non-TSOd version) has been working great for 18 months since my first flight, and with a belly mount 45 degree whip antenna I get great reception and (from what ATC and other planes tell me) excellent transmission quality too. For the $$ I think the ICOM is a great buy.
 
iCom came out with the non-TSO'ed version first. Then a few years later they started selling the TSO'ed version for production airplanes. At that time, they said the only difference was the paperwork. (Maybe George can dig up that news release. He seems to have better luck finding things than I.)

Things may have changed since then but I doubt it.

The ICOM, TSO'ed or not is a great value. Now, if we could only get them to make a NAV/COMM. I talked to one of their reps at OSH last year, who said they were considering it, but needed more "interest" for their customers.
