Hi All,

I stopped by the Notarizer this AM to get my 8130-12 notarized and she wasn't exactly sure what to put into the "Notarization Statement" box. Rather than taking a chance, thought I'd ask just to be 100% sure. Is it just her stamp and signature? If a "statement", what kind of a statement? Something else?

Each state has it own laws but basically the substance of the statement is that the notary public is certifying that you (the signator of the document) actually appeared before the notary on a specified date and signed and/or acknowledged your signature before the notary. If the document doesn't include the language a notary will usually have a stock paragraph in a handbook.
