Default Control reversal - Very sad accident
I've been following this accident as one of the pilots killed was a friend and classmate of mine from college.

NTSB has published a preliminary report and it looks like the plane's aileron controls were reversed.

"Flight Controls - Free & Correct" is on my checklist...

Edited to add: Though to be fair, I'm not sure they could even see their wings from up in the cockpit.
