Where do I get a fuel servo adapter?

I am in need of an adapter plate for connecting the approx. 4" sump opening to the 2" side of the Bendix FUEL SERVO.

Engine: Lycoming IO 360 A1B
Fuel Servo: Bendix

Does anyone know where to pick up an adapter plate? Also, should I get an angled adapter or just a straight adapter. I am using a Sam James Cowl. Thanks
