registration question.

I am filling out the paperwork to register my RV-9A and have a couple of questions. On the 8050-88 Affidavit of ownership form the first line is

U.S. Identification.

I am assuming this is for an N number if you have reserved one. I typed in the blank

N 9JW (reserved) in the space provided. Is this correct?

Also since I already have a reserved N number do I just send the $5.00 fee or the $5.00 plus the $10.00 for a special requested number. I already paid for the reserved number so I wouldn't think I woul need to send the extra $10.00.
