Kit Shipping Costs Spreadsheet/List (Contribute Info!)

I've been doing some pretty heavy searching for shipping costs for these kits, and I've decided to try to assemble an RV shipping costs list, which I'll post for public use as it's being completed.

So, if you guys would be willing to help, please e-mail me (mike *at* 41past *dot* com) or PM me the following information for each kit shipment you've received. If you've received several, please complete this info as many times as necessary. I appreciate it, and I'm sure other builders will like being able to quickly reference shipping costs to their approximate location.
