Default Amateur-Built Fabrication and Assembly Checklist

My DAR stated that for my RV-6A I am required to fill out the Amateur-Built Fabrication and Assembly Checklist(2011). I thought that this form was for kits that were not already on the 51% rule.

Am I incorrect? Does everyone have to fill out this form for their RVs?

Just curious.

The form is required if any of the items below exist;

1/ The kit is not on the approved list.
2/ The design has been modified from the original kit.
3/ If commercial assistance was used. or
4/ If the inspector has reason to question the 51% rule compliance.

I am to assume that the RV-6A is on the approved list? I can provide evidence for items 2-4.
