PCU-5000X order form - how to fill out?

Looking for some help in filling out this form to order a PCU-5000X governor. I will be installing an a stock Van's IO-360-M1B engine and Hartzell BA prop.
Data needed:
- Governor Drive Ratio (I assume this will be 0.866:1)
- Control Lever Angle (I assume I can change this as needed)
- Control Lever Rotation (Clockwise or Counter-Clockwise)
- Pressure to increase (Yes or no?) for non-counter weighted props (which I assume I have).
- Pressure to Decrease (Yes or no?) for counter weighted props which I don't think I have.
