Default FAA Form 8050-88 question

What did you put on line one for US Identification? And what order did you put your name on line two, last, first, MI ? and this needs to be notarized? Thanks.

I just mailed all this a couple days ago. I had reserved a number and that's what goes on the US Identification line, if you've reserved one. On the name, it appears that you can't go wrong with Last Name, First Name, MI That's what is on my Bill of Sale from Vans, my id plate on the plane, and everywhere else. But, I'm sure I screwed it all up somewhere...

N number, ok, thanks, so on the engine question, is the FAA that concerned with us experimental folks? my engine is now EFII equipped so is there any relevance to my model and serial # now? do I just make one up?

My engine had a data plate, so I furnished all the info thereon. It's a Titan IOX-375.

I think I remember that if it's not a certified engine, you can put what you want on that. As I recall, Mel had a post in re: this in the past.
