Example AC 20-27G Appendix 8

I'm curious what a finished AC 20-27G Appendix 8 looks like. Can someone post one that they submitted to the FAA?

Specifically, I'm wondering what it looks like when you've received commercial assistance? But even if you didn't use commercial assistance, I want to see how you split the percentages of fabrication with the QB kits.
