DAR/Paperwork question: Eligibilty Notarization - attached Document
This is a procedural question and I'm not sure what to do about it.
I took my FAA form 8130-12 (Eligibility Statement - Amateur Built Aircraft) to the local Notary Public at the UPS store on the corner and he had an issue with section IV, the Notarization Statement.
He said he couldn't sign the notarization statement as is, because Section IV doesn't contain the following language:
"Subscribed to me ans sworn on this____ day of _____, 20__,
by (1)______________
proved to me on the basis of satisfactory evidence to be the person who appeared before me.
Signature_________"
He said that CA requires that language to be in any document that he notarizes or he has to attach a California Jurat With Affiant Statement containing that language, and "see attached statement" written into Section IV of the Eligibility form.
I then asked him, "OK, now how are you going to connect this form to this attached paper with your stamp on it?" His answer was to stamp both pages with half of the stamp on the 8130-12 and the other half of the stamp on the attached form as well as a full stamp.
While perfectly legit, this sounds hinky to me, and likely to trip me up when it comes to my DAR inspection or FAA acceptance of my paperwork.
How do I handle this? Find another notary? put the required language in the 8130-12 in really small print? Use it as is?

I agree it sounds hokey, but other notaries have brought this up.
I have had several 8130-12s done the way you say, and so far FAA has not kicked any back.

When I make my DAR appointment I'll ask him as well, but if you don't know of any that have been kicked back, this will probably be OK.

Find a friend who has a notary, the one I used saw this also but put her stamp on it and signed it... There was no problem during the certification

First off, I am not a Notary but I am not surprised you are having issues.
I have had occasion over the years in California the need to do several different kinds of things that require a notary to sign. I bet on about half of them the local Notary said they could not do it per the California Notary laws. A couple of them were some sort of Notary Instructors and both said it wasn't all that uncommon for forms that were not generated within the state to not be able to be signed or not easily signed. No clue how true that is, just relaying what I have been told on more than one occasion.

I used to be a Notary years ago for my work. The only thing I was notarizing was that the person signing the document was the person named in the document and produced identification in my presence and signed the document in my presence. The jargon and legalize in the document was not my concern. I just had all the required paperwork notarized for the registration and DAR inspection of my -10 done at my bank that also provides notary service for its customers. Both notary's I dealt with handled it the same way I did 30 years ago.

I just looked at mine, in California, and all it has at the bottom is a stamp with the notary's name, commission number, expiration date, and their signature.
Done at the local UPS store.
If you have time, I'd say just go somewhere else...

Notary By Attachment
I just looked up my registration documents and my 8130-12 was notarized by attachment exactly the way you have described (May, 2010). I had absolutely no trouble with any of my paperwork when I submitted them to the FAA.
