starter issue

I have a RV-12 with 912uls about 20hrs total. today after shutting down and restarting the starter just whirred with no engagement.
I removed the cowlings to find the 2 nuts holding the starter on were gone and despite the clamp it had moved about 1/2 of inch thus not engaging.

First thing I'd do would be a complete aircraft inspection. Check torques on every single bolt/nut. Was the aircraft thoroughly inspected by the DAR 20 hours ago or was it a "paper" process?
 
went through and torqued everything on the engine. must have missed these as starter came attached.
inspection was by Faa-very thorough
 
Don’t feel bad I had the same issue at about 300 hours. Installed new lock nuts and they have been fine for the last 150 hours.
